Intensive Surgical Training
Two full days of intensive, alternating hands-on sessions guided by both masters. Participants will switch instructors on Day 3 to ensure comprehensive exposure to both surgical philosophies.
Four flap designs in the aesthetic zone, dependent on patient anatomy and detailed diagnostic measurements, will be described. Participants will master site-specific soft tissue manipulation and learn to mitigate complications in the aesthetic zone during high-volume augmentation, with a specific emphasis on the thin phenotype.

  • Complication Prevention: Strategies to avoid scars, recession around implants/adjacent teeth, and papilla loss.
  • Individualized Protocols: Customizing the incision, flap design, manipulation, and suturing based on patient phenotype using specially designed micro-instruments.

In the realm of advanced surgery, the difference between a functional outcome and a masterclass in aesthetics lies in sub-millimetric details. Day 4 of AMS 2026 brings together two titans of regenerative surgery, Dr. Giovanni Zucchelli and Dr. Marius Steigmann, to dissect their distinct philosophies on tissue handling. This is a deep dive into the biological logic and technical precision that govern the future of the gingival margin.

I. Papilla Manipulation

The Interproximal Biological Interface

  • Dr. Giovanni Zucchelli : The papilla is elevated split-thickness, keeping the blade parallel to the external surface (superficial split thickness) in order to minimize the thickness of the papilla itself and maximize the thickness of the underlying connective tissue (anatomic disepithelized papilla). This is the area that provides the blood supply for the stability of the flap advanced coronally.
  • Dr. Marius Steigmann : The papilla is left in position with a semilunar 90-degree incision at the papilla base, only in the epithelium first. Then, with a curved blade in the KG and/or a splitting instrument in the mucosa, it is separated from the connective tissue or periosteum; this is the area that provides the blood supply for the papilla and is used to reposition the flap to the initial position. Disepithelization is not necessary.

II. Flap Mobilization & Passivation

The Biomechanics of Tension-Free Closure

  • Dr. Giovanni Zucchelli
    Mobilization and passivation are obtained through two different incisions:
    • The Deep One: (Blade parallel to the bone), which is very limited in the apical direction; its sole role is to detach muscle insertions from the periosteum, which is left protecting the bone.
    • The Superficial One: (Blade parallel to the external surface), which is extended apically; its role is to remove the muscle from the inner aspect of the alveolar mucosa. Result: The alveolar mucosa, rich in elastic fibers and free from muscle, allows for the advancement of the flap.
  • Dr. Marius Steigmann
    Flap mobilization and passivation are obtained with mucosal detachment, leaving only the periosteum on the bone. The mucosa alone, and then apically the mucosa and attached muscles, are detached from the periosteum. In case of inadequate mobilization, additional soft tissue manipulation in the mucosa only is necessary.
    • For GBR: The periosteum is detached from the bone to allow for the placement of bone grafting material.
    • For Mucogingival Surgery: It can be used to fixate the soft tissue graft.
